Nurse Practitioner – Internal Medicine (Skilled Nursing Facilities)
The Polyclinic is one of the largest multi-specialty clinics in the Puget Sound area, with more than 175 primary care and specialty physicians in most areas of medicine. Established in 1917, The Polyclinic's mission is to promote the health of our patients by providing high-quality, comprehensive, personalized health care.

We are currently seeking a full-time Polyclinic nursing home mid-level provider to provide short and long-term medical management to primarily geriatric population, in skilled nursing and long-term care facilities.

QUALIFICATIONS:
Education: Bachelor of Science in Nursing; Master of Science in Nursing; Completion of Nurse Practitioner Course of Study. *Open to Physician Assistant candidates for this position as well.

Certification/Licensure: Current Washington State nursing license and ARNP licensure with prescriptive authority.

Experience:
1. Three to five years of professional experience as a nurse practitioner in Geriatric Care.
2. Clinical expertise in cardiac, pulmonary, neurological, chronic condition management and other multi-system illnesses.
3. Working understanding of Medicare regulations and management of managed care populations.
4. Palliative care and end-of-life medical management experience preferred.
5. Experience with HMO and insurance plans preferred.
6. Experience with electronic medical record system(s) desirable.
